This Notice of Privacy Practices applies to Psynergy Services, LLC ("Psynergy," "we," or "the practice"), including its clinicians and workforce members. We are required by law to maintain the privacy of protected health information (PHI), to provide you with this Notice, and to notify you following a breach of unsecured PHI as required by law.
Treatment. We may use and disclose PHI to provide, coordinate, or manage your care, including with other providers involved in your treatment and for telehealth services.
Payment. We may use and disclose PHI to bill and collect payment from you, your insurance plan, or other responsible parties.
Health care operations. We may use and disclose PHI for quality improvement, training, licensing, auditing, business planning, and customer service.
Appointment reminders and communications. We may contact you by phone, text, email, or patient portal about appointments, scheduling, and care-related logistics, consistent with your preferences and applicable law.
As required by law. We may disclose PHI when required by federal, state, or local law.
Public health and safety. We may disclose PHI for public health activities, to prevent or lessen a serious threat to health or safety, or as required for abuse/neglect reporting.
Health oversight. We may disclose PHI to health oversight agencies for audits, investigations, and inspections.
Legal proceedings and law enforcement. We may disclose PHI in response to court orders, subpoenas, or other lawful process, and for limited law enforcement purposes as permitted by law.
Workers' compensation. We may disclose PHI as authorized by workers' compensation laws.
Business associates. We may share PHI with vendors who perform services for us (such as billing or EHR) under agreements that require them to protect PHI.
Other uses and disclosures not described in this Notice will generally be made only with your written authorization. You may revoke an authorization in writing, except to the extent we have already acted in reliance on it.
To exercise these rights, contact the practice using the information below. We may ask you to submit requests in writing.
If you believe your privacy rights have been violated, you may file a complaint with Psynergy Services, LLC at the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights (www.hhs.gov/ocr/privacy/hipaa/complaints/). You will not be retaliated against for filing a complaint.

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as PHI we receive in the future. The current Notice will be posted on our website and available upon request. Effective date of this Notice: August 25, 2026.
